Windows 10: 100% Disk Utilization even after I Full Reset

Even after I fully reset my pc disk usage is 100%. I have tried every possible way available on Internet.I've done a fresh reset to factory settings and it's still the same.

100% Disk Utilization even after I Full Reset

I've been searching every possible answer to my problem and tried every possible solution how to solve my 100% Disk Utilization problem.

I even Full Format my laptop but still nothing works.

Worst case scenario is after i reformat my laptop, it's getting worst, just running one program makes my Disk Utilization to 100% and then froze for a few minutes and drop disk utilization to normal, when i'm going to use the program it starts to go up again
till 100%

Take note, it is full formatted a few minutes ago .

I just post my dxdiag details here for you to know what my problem is and your help is pretty much appreciated.

 

100% Disk Utilization even after I Full Reset

Before i run any program all is responsive, but not just like before there's some delay.

A. After i run any program (bluestacks, google chrome, cmd) my utilization disk starts to go up to 100% and after sometime it lags or freeze and after a few minutes it will go back to normal.

B. After the lag/freeze, let me say i got two open programs and i want to switch to another, the utilization disk again shoots to 100% and lag/freeze.

Take note: My laptop is in fresh format and just got all my drivers updated even OS is updated. Before fresh format it's not like this. and i've done plenty of formatting, resets, factory reset any term for the word to have it like brand new.

My laptop runs slow and I tried hard resetting it and still it has 100% Disk Utilization and still runs slow.
